You may qualify if you used a FreeStyle Libre 3 or Libre 3 Plus sensor in 2025, have Type 1 or insulin-treated Type 2 diabetes, and suffered a serious medical emergency — DKA, severe hypoglycemia, or hyperglycemic hyperosmolar state — that required medical treatment.
